Install aweber form

How do I install an aweber form on my blog. The documentation is not helpful about this.

There is a way to choose aweber in the newsletter set-up, but nowhere I can see to put in the proper code. Also, when in the response it wants you to put [email protected], is the domain mine or aweber’s? (I have set up the address on aweber, but I don’t know what to put here if I could find it which I can’t.)
