 * When I installed and tried to add a new page in the visual tab all the attributes
   were gone – deactivated the plugin and everything went back to normal. any suggestions?

 * What do you mean “add a new page in the visual tab all the attributes were gone”?
 * What theme are you using?
 * Does this only impact a specific plugin? Page? Post? Theme setting?
