To debug this further, please go to “MC4WP > Others” and enable logging to “Everything”. Once this is done, please test the form again and then share any errors listed at “MC4WP > Others”.
When sharing the error log, please remove any sensitive data.

I did as you suggested and retested the WPForm contact form. The bug log reports no errors. Integration with a WPForm form only requires the addition of the check box to subscribe and linking MC4WP to my Mailchimp account, correct? I have linked my account using the API key and it seems to work since MC4WP is pulling my info from Mailchimp. I added the button to the form so it seems as though the NAME and EMAIL fields should be pulled into my Mailchimp list, but nothing is imported/added when I test the form. I can’t help but feel like I’ve missed a step?

Can you please go to “MC4WP > Others”, and select “Log Everything” and test again? I just tested the form but did not get any double opt-in email.
Also, to send the name field, you will need to add additional code and example of which you could find at https://github.com/ibericode/mc4wp-snippets/blob/master/integrations/send-name-field-from-wpforms.php.
