 * Hi, when I try to create new form I get this notice.

 * Also, my comment form on frontend disappears when I install the plugin…
 * I tried to disable jquery in settings, disabled all plugins to see if there is
   a conflict, but still the the notice is here.
 * Do you have maybe an idea why is this happening? Thanks!
