Issue With Forminator Plugin

Hi,
We are facing issue with the forminator plugin. I checked error logs and noticed forminator plugin fatal error:
Unable to capture the lead and also banner images are missing, testimonial slide not working and videos also missing.

I see errors in the browser console related to the Forminator plugin. Can you manually update Forminator plugin core files and folders via FTP in this folder: /wp-content/plugins/forminator and see if that helps?

I can’t see any Fatal errors when submitting the form but some Javascript errors in browser console logs is visible which are related to the form validation.
The issue might be due to a plugin conflict issue, could you please try running a conflict test by disabling plugins and switching to a default theme?
You may take a look at this document to learn more about how to do that, I would suggest creating a backup before starting the test.

I disabled the Forminator plugin, and the page started working fine. It seems that the plugin is causing the issue. Could you please advise on how to resolve this?

That indicates there is a plugin/theme conflict but we can’t tell which plugin is the root cause of the issue without seeing the site configuration and error logs.
If you wouldn’t mind please first go to wp-admin => tools => site health => info and copy/paste the information to PasteBin or another free online storage and share the link here.
Also please share your PHP error logs, you may share the whole file or share the exact error you found about Forminator.

I checked the mentioned error, however, we haven’t observed any similar errors reported at our end. Additionally, the plugin is working fine at our end.
The fatal error is occurring because the function, strlen(), expects a string but received an array instead.
I checked the list of active plugins from the Site Status that you shared, however, we haven’t received any conflict reports on any of them at the moment.
I see that you are using a custom theme and have WPCode Lite plugin active on your website. It is likely that a custom code that you are using on this website is causing the issue.
To help you resolve the issue, it is essential to find what exactly causes the conflict.
Would it be possible to create a staging website so that further tests can be performed without disturbing the live traffic? A staging website is a copy of your live website in a new directory on same server using a separate database. Please perform a complete conflict test on the staging website and update us here with the results so that we can help you further.
