javascript – form not appearing on website

Our donation forms are suddenly not working.
To create our online donation forms, we copy javascript from a form created in Online Express through Raisers Edge / Blackbaud.

We take this code and put it into our WordPress pages.
Around August 14, this donation form ceased to appear on our website – it was normally visible and appeared on our main donation page and was linked to a number of donation buttons.

Normally, for example, when you go to the link listed above, you would see a donation form under the red banner. Now it’s just a white space – it just disappeared. And, every other place on our site where we have a “donate now” button, we have the same issue.

We have checked with Blackbaud, who confirms that the javascript is accurate. In the preview from their site, the form is working.

We have checked with our web host (GoDaddy) – there are no caching issues, firewalls, etc., on their end.

We have used the javascript from Blackbaud and the various pages in WordPress for years without any issues. Now – suddenly – this javascript isn’t working and we’re without the ability to accept donations online.
