 * I’m trying to figure out how to make a custom plugin that would allow me to create
   a form and use images as the radio buttons. also would like to include “conditionals”
   for future use.
 * I’ve searched all over the net and theres nothing that fits my needs
    help!!!!
   driving me crazy!!!
