 * Can you please let me know whether you are able to build the form in the form
   builder or not? I mean, let me know whether you are facing any issues in the 
   form builder or not.
 * Also, please elaborate on your query a bit more so that it would be easy for 
   me to dig into the issue and help you fix it.
 * Lastly, I want to suggest you deactivate all your plugins except Everest Forms
   and see if the issue exists or not. If there are no issues then you can activate
   your plugin one by one and then check at the same time so that you can know if
   there’s a plugin conflict that could have been causing the issue.
