# List Form has extra code??

 * Hello. I have a client who uses LeadPages and wants me to incorporate WPNewsman
   with it. LeadPages has an html option and worked fine BUT there are fields that
   I did not put on my form & are showing when I put the code in.
    First this is
   the form I am referencing. I only ask for First Name and Email. [Form Pic](http://kelleeflesch.com/pic2.png)
   Here is the screenshot of what comes up when I put the code the form gives me
   into the html section. [HTML Form](http://kelleeflesch.com/pic2.png) Where are
   these bottom 2 fields coming from? If you look on the left I can delete the bottom
   2 fields but when I the form doesn’t work. Where do I go and how do I change 
   this code?? Thanks, Kellee

 * Sorry, you posted the same screenshot 2 times. I do not see 2 additional fields
   you are talking about. Please, submit the screenshot with 2 unnecessary fields.
