    i have a Movie Trailer Site and users every day send me an email for requesting
   a movie trailer and now i want to open a “Page” at wordpress so users can send
   me the movie details example: ———————– **Movie Title:** [box] <— enter the movie
   tittle! **Comment: ** [box] <— enter a comment about the movie! **Your email:**[
   box] <— enter your email address! ———————– Only a plugin or a HTML document as
   a Page [not a widget] similer the “Contact us” plugin !
