Mail Chimp Subscribe button mis-aligned

I’m trying to install this pop-up for use with a Mail Chimp email list, but for some reason the auto-generated Mail Chimp code is getting messed up somewhere along the way.
For some reason, no matter what I try, the “subscribe” button ends up way over on the right side and running out of the box, when it’s supposed to be left aligned.
Anyone have any idea why this is happening? 🙁
