 * Have you tried disabling _**Merging of JavaScript files**_ option at _**WPO >
   Minify > JavaScript**_? If not, please do and let us know if that helps.

 * I tried everything but the problem still exists. I had to turn everything off
   to get my form to show up on the site.

   Sorry to hear that this issue persists despite all that.Unfortunately, I 
   am unable to replicate it as I don’t have a copy of the cost estimation plugin
   in question.
 * You might want to consider excluding JS and CSS resources belonging to the cost
   estimation plugin from minification. This can be done at _**WPO > Minify > JavaScript
   > Exclude JavaScript from processing**_ and its equivalent under the CSS tab.
