 * Getting locked out even though I’m on a whitelist. It even say I triggered a 
   lockout but I’m on the white list. But I do get locked out.
 * Browsing through my media library triggers a lockout also.
 * Problem happened when I updated to 3.9 / updated my theme (X) and upgraded to
   4.0.25 but 4.0.27 did not fix.
