 * Hi JBWorks,
 * The free version of the plugin only allows one form, which either subscribes 
   to one or multiple MailChimp lists.
 * If you want to create more forms which subscribe to different lists (or the same,
   for that matter), please consider [getting the premium version of the plugin](https://mc4wp.com/).
   It allows you to create as many forms as you like, each with their own settings(
   lists, messages, styling, etc..).
