Need correct code for Subscribe bar to function

Where can I find the correct code to put the “Add form embed code” for my top bar email subscriber campaign that connects with the Icegram email subscriber lists on my site? It currently shows;

<form action=”” method=”post” id=”demo-embed-subscribe-form” name=”demo-embed-subscribe-form” class=”” target=”blank” >

<label for=”NAME”>Your Name… </label>
<input type=”text” value=”” name=”NAME” class=”” id=”NAME” required>

This obviously needs to be changed from Demo, I just don’t know what needs to go there or where to find it… Thanks.
