Nested select field/dropdown not working

I have some conditional fields set up in this form. With some of the select dropdowns, if you choose “Other Insurance”, a new text field drops down for people to fill in their information. However, once that option is selected, regardless of whether someone adds data to the text field, the dropdown menu stops working.

Select “No”
Under “What Type of Insurance Are You Interested In?”, select “Personal Insurance”
In the following dropdown, select “Other”.
Now try to click on the dropdown to select a different option than Other. The select field no longer works.

The same thing happens with the “How Did You Find Us” field – select “I was referred to you.”

Any thoughts on why this might happen? It was happening on version 1.8.X, but I also updated to the newest version and it is still broken.
