# New client input form

 * I am setting up a business as an accountant. My site is still in maintenance 
   mode, [http://www.padamwalburn.com](http://www.padamwalburn.com)
 * I have a contact us form, which I installed via a plug in, but I am trying to
   write a “new client input sheet”, which needs to have drop down boxes for things
   like “title” and “business type”, and depending on the answers will bring up 
   additional questions. So if someone selects “partnership” in the business type,
   it then asks for the names of the partners etc.
 * Is there a plug in that does this, or how do I go about writing it myself?
 * I only have basic computer skills, but am really please with what I’ve been able
   to do with wordpress.
