 * I went to a popular image harvesting site and typed in mydomain.com/blackhole/
 * In wordfence, I see the access, but the blackhole plugin did not add anything
   to the bad bots list. In fact, the list is blank.
 * How can I solve?

 * The Blackhole trigger link has a nonce/number appended as query string. Without
   it, the link will not work. You can find the correct link in the source code 
   of your pages, assuming everything is set up correctly. Also note that the trigger
   link nonce changes periodically.
