Only displaying Submit button

    Hello,
    Running WordPress 4.7.2 with latest version of Pirate Forms and Zerif Lite template.

    For some reason, when I use the short code, it only displays the submit button, and not any other parts of the form.

    My server is running at: http://162.243.1.110 (scroll to the bottom)

    Any thoughts?
