Same here: tried a test survey but got a 404. Seems an issue with the permalink, which becomes www.whatever.com/lxt_jast/test-survey/, if pretty permalinks are enabled.
With WP’s default permalink settings it works though.
Suggestion: make the permalink structure for the survey post type editable for users (‘lxt_last’ is not something I would want to use).

The bug about permalink has been solved, you need download the package and re-active the plugin.
