 * Does this support paginated posts?
 * I’ve been looking everywhere for a table of contents plugin that can appear in
   a widget, work with paginated posts, and highlight the current page.

 * Hi,
 * I have done by development of TOC+.
 * It can hold also to build toc for all displays generated by queries on posts (
   lists of posts) or any automatically built documents. Then you can for example
   produce a widget with the list of the posts or their excerpts which is produced
   and linked to.
 * It functions for me.
 * I will publish a first version with only paginated posts.
 * I would to do it with agreement of the author of TOC+, I don’t got answer since
   two month.
 * I am looking for the best way to make available this update.
