Thanks for allowing access, I have tested the form on two of my installations and I’m afraid that I can’t replicate the issue there so it seems there’s something specific on your installation causing this.
Can you go to Forminator Settings and ensure that you have correct PayPal details entered, double check that you haven’t switched Live with SandBox ones, it happens 🙂
If that doesn’t help I would suggest doing a conflict test by disabling your other plugins and switch to default WP Theme to see if the form works after that.
If it does, enable them back one by one until the issue is back and that should tell us where the conflict is coming from so we can debug it further.
I suggest doing this test on a staging site, or if you don’t have one make sure to create a full site backup before testing on live installation.
