 * I trust you’re doing well!
 * I apologize for the delay in this thread. I could not replicate this issue. Does
   it happen upon plugin activation?
 * Please try to re-install the Forminator plugin. Download a fresh copy and access
   your site’s folders via FTP or a File Manager, go to `/wp-content/plugins/`, 
   replace the current `forminator` folder with a fresh one and see if the issue
   is still there.
 * Please let us know how it went!
