When installing Hierarchy on my local machine with WP_DEBUG set to true I get the following:

Actually, please disregard this.

My issue was due to an xml import (through WP-CLI) for which the corresponding posts didn’t have authors assigned.

I think this is still Hierarchy’s responsibility. Opened as an Issue: https://github.com/jchristopher/hierarchy/issues/13
