 * Is there a way to use the functionality of this plugin within a custom template
   page, rather than through the wordpress admin?

 * Yes, i find a way to do that.
 * 1.) Define your Filled in form as normal.
    2.) Copy your form-HTML-code (with
   the correct id=”filled in form name”) in a post or page and show it on the front-
   end. Then grab the generated form-HTML-code as shown. Take this and put it in
   your template page.
