    with WP_DEBUG set to 1, the plugin show notice alert in admin page.
 * “Notice: Undefined index: submit in …/content/plugins/fitvids-for-wordpress/fitvids-
   for-wordpress.php on line 66”
 * And
 * “Notice: Undefined variable: checked in …content/plugins/fitvids-for-wordpress/
   fitvids-for-wordpress.php on line 94”

 * You need to check if those $_POST[] variables are set first.
