 * Sorry for the late reply because our location just got a typoon.
 * The issue is not the plugin but in your WordPress theme or other 3rd party plugin
   that is overriding the style of the form. you need to to check with a developer
   to troubleshoot it.
 * Also we have refunded you already long time ago because we could not troubleshoot
   it because of the typhoon.
