Plugin stopped working

This plugin worked great when I first installed it, but suddenly stopped working.
I installed it in order to change my permalinks from day and name (ie, http://domainname.com/2014/07/21/sample-post/)
to post name (ie, http://domainname.com/sample-post/)
Now all old links are generating 404 errors. 🙁
I’ve tried disabling all plugins, and only enabling Change Permalink Helper, but that didn’t help.
I’m running WP 3.9.1, using the Divi theme by Elegantthemes.
Please help! 🙁
