[Plugin: WordPress Form Manager] Form layout

    I would like to know how to create a form in “table format” by using Form Manager that means it presenting as:

    Date/Available Time
    Mon “checkbox”13:00-14:00 “checkbox”14:00-15:00 “checkbox”15:00-16:00
    Tue “checkbox”13:00-14:00 “checkbox”14:00-15:00 “checkbox”15:00-16:00
    Wed “checkbox”13:00-14:00 “checkbox”14:00-15:00 “checkbox”15:00-16:00
    Thur “checkbox”13:00-14:00 “checkbox”14:00-15:00 “checkbox”15:00-16:00

    a table format with rows and columns and which allows multiple selection as now i can only make form in one column.
