# [Plugin: WordPress Form Manager] Form throws "Unknown column '0' in 'field list&

 * Form Manager 1.6.38 on WordPress 3.4.2.
 * Used the “Insert Saved Form” link at the top of the Edit screen to copy a form(
   ID = 3) to a new instance (ID = 4), then edited the fields copied across and 
   added a couple of extras manually. All worked fine until testing the form, and
   it threw an “Unknown column ‘0’ in ‘field list'” SQL error.
 * Looking into the database tables, I noted that the type prefix on all the copied-
   over unique_name identifiers in the tables wp_fm_items and wp_fm_data_4 were 
   all missing. Changing the unique_name identifiers manually through SQL editor
   to the right format in wp_frm_items and wp_fm_data_4 fixed the problem and the
   form submitted OK with emails sent as per normal.
