If you had a certain page at your site that is the only page allowing comments, the default WordPress “Recent Comments” widget could then display a menu of those comments. If you have comments enabled for more than one page, you might be able to find a plugin that would display comments selectively.

I am not able to walk you through all the specific details for doing exactly what you want at your site, and adding a phone number would be something more like for a contact form than for a comment.
Dashboard > Settings > Discussion is where you can control comments for pages, and then Dashboard > Appearance > Widgets is where you can put the standard “Recent Comments” widget in place. If “rolling upward” means you want titles or excerpts displayed and constantly changing like a ticker tape, something like that would require searching through plugins until you find one or more than can do that.

Dear masters,
I would like create a prayer request (comment) form in widget. And after some one filled the form, the prayer requests (comment) can appear in widget and moved rolling-upward.
