 * Hello,
 * I installed your plugin correctly,i followed instructions as on the screenshots,
   but when i publish or want to preview the questionnaire,it links me to a blank
   page. I have all the questions set up,i double-checked and everything seems to
   be in order but it still doesn’t work.
    Need urgent help!

    Thanks for installing this plugin. Please teach me about: * your browser
   and version. * The version of plugin. and if you don’t feel bother(and there 
   are no security matter), please post your meta data here. (you can get meta data
   of questionnaire by using ‘Custom Field’ meta box. see ‘qstnr_metajson’ value)

 * If you see just a ‘Blank page’, it may be a JavaScript problem.
    1. No JavaScript
   execution allowed on your browser. 2. There are some incompatibility with JavaScripts
   I using with the browser you using.

 * Hi, I’m Maurice from BBC Media Action. we might use “Questionnaire” in one of
   our online training. but have couple of questions about it first.
    sorry, but
   is it the right place to put my questions? I would ask you whether the results
   are tied to an individual user name or not – and if they can be retrieved that
   way.
 * That’s the kind of functionality I want ask for:
 * – Pre and Post questionnaire (4 – 5 questions each) – per unit or module.
    – 
   Each questionnaire tied to an individual user name – Results can be individually
   and collectively downloadable in a report on excel sheet
