Problem with checkbox

Hello,
I created a form – title [fu-upload-form form_layout="post" name="form1" class="profilvloz" title="Vložte profil"]

and under:

[checkboxes name="foodig" class="checkboxes" description="Pick a fruit" values="value:Description,124:Banana,cherry:Cherry"]

But when I submit this form, information from checkbox doesn’t show in the post (no foodig custom fields with checked items)

Please, can you help me?
