(We will be a bit limited in trouble shooting modified form code, but we can certainly take a look and help as much as we can.) 3) Where are you seeing the error you mentioned? 4) Any details relating to your workflow would also be very helpful. Do you happen to have any required fields in your list? If so, it may be that your connected list simply needs to be updated in WordPress. Your list can be updated for the plugin by logging into WordPress, navigating to the Plugins page, and then clicking “Settings” for the MailChimp plugin. From there, please click the “Update List” button shown to the right of the list drop down. That will refresh your list settings in WordPress and that should resolve the form issue you are seeing.
