Remove documents from page

I’m sorry if this has been addressed before, but I didn’t see anything related in the other topics. How do you remove a document after adding it to a page? I’ve noticed that the code for each media file disappears soon after adding it and I’m not sure where to go from there. Any help is appreciated!
