    i have developing my site and stuck at claim form. i want form that runs
   through conditions like this one _[ redundant link removed ]_. i want exactly
   same type of form. Please can u guide me through it.

 * The form seems to be built using a custom plugin. We will not be able to help
   you with this problem here on WP.org as the code for the form is not provided
   from the downloads available on this site.

 * As I said previously we can not provide support since you are using a custom 
   plugin. If you want to move the form into another forms plugin I’d suggest using
   gravity forms. In which case you would be needing to contact their support as
   they do not provide their code through WP.org.
