    You must have a problem with your theme or your WordPress.

    Did you try to disable WPS Hide Login, if the form reappeared?

    Same problem here. It did not work until I disabled this plugin, but in this case it says “Your password reset link appears to be invalid.”

    Hello, you must have another plugin that WPS Hide Login that is problematic because with the latest version it works properly.

    Have you tried disabling other plugins?
