Resetting default check boxes on form?

Hi Folks. I’m having a challenge with check boxes that seem to be checked by default every time I add something new to a form (for purchasing items). I’ve searched through the WordPress site here but can’t seem to understand the solutions that have been given to others. I’m not very tech-savvy. *Sigh* Any assistance would be greatly appreciated thank you. Does this have anything to do with my licence for Gravity Forms expiring? (Something I didn’t read about in my website designer/host’s fine-print).
