 * We investigated the issue and it looks like it is not caused by CaptainForm. 
   There are two forms on your website that work correctly: [https://www.kolayflooring.com/contact-us/](https://www.kolayflooring.com/contact-us/)(
   the one on the page and the one in the footer).
 * The error seems to be coming from our plugin because we catch all the errors 
   that appear on the page and display them at the end. We are doing this because
   any JS error that occurs on the page can cause our forms not to be loaded correctly.

   The error could be caused by an external script that is loaded without the “crossorigin”
   attribute.

   your scenario, like the URL of the page where the form that is not loading is
   published. You can send us more details in a support ticket, so that you don’t
   share this information publicly.
