SEF URL

I downloaded the sign-up sheet plugin.
It works great, but the URL is an awful mess. Is there a way to fix this using the plugin or do I have to use an outside plugin?

That’s the URL. All my other URL’s are SEF-friendly, but now this one has the ugly ending: child-care-services/?sheet_id=1.
How can I fix this?
