SendPress Form Widget: won't save form selection

Hello –

When using the “SendPress Form” widget, if I select a form that I created under “Form to Display”, it always reverts back to the “Default Signup” form after Save. Even if I select the “Default Manage Subscriptions” form, it reverts to “Default Signup”. Seems it is not accepting any selection other than the default.

Any advice on how to make this widget accept a form selection? Thanks in advance.
