 * I have selected to add the optin form using the optin short code. I have added
   it on the home page.
 * On my blogs now I have the actual option short code visible [optinform] above
   the post header. I didn’t add them on the blog page

 * I believe it may be the Divi theme causing the trouble here. Perhaps the page
   builder is stripping out the shortcodes.
 * Did you try getting in touch with the theme developer Elegant Themes? I think
   they will be able to help you out much faster, as they know how their product
   works 😉
