 * The shortcode [contact-form] is fairly generic and may conflict with other contact
   form plugins. I noticed that your plugin wasn’t showing up on my site, and I 
   figured out it is because it was being preempted by the contact form used by 
   Jetpack, which comes preinstalled by default on many WordPress installations.
   I ended up getting Clean and Simple by using the editor to change your shortcode
   to [clean-and-simple-contact-form]. You might want to think about changing the
   default shortcode as well.

 * Glad you like the plugin. I use jetpack too, it’s a great plugin.
 * What you could have done instead was to deactivate jetpack’s contact form module(
   assuming you didn’t want to have two on your site). This can be done without 
   having to deactivate the whole plugin.
 * When I named my plugin I realised it was a generic name but I prefered it because
   it is intuitive for users, and also (perhaps optimistically), that users will
   have only one contact form plugin installed at one time.
 * What I might do is to implement an alternative shortcode such as the one you 
   have suggested. That way, users can still have an intuitive name but the alternative
   caters for times when a conflict could occur.

   as I intend to keep on improving.
