 * Alas, the options are all being ignored — only the E-Mail form field is shown.
   We’re somewhat stymied; what is going wrong?
 * It’s WordPress 5.1.1.

 * Hello,
    any update on this? Got same situation. Plugin is connecting to RapidMail,
   can take correct list, but on WP side it doesn’t generate form according to shortcode.
   At widget area is same on Email and Subscribe fileds are displayed to check.
