“Something went wrong, please try again”

Hustle, Version 5.1.2
When the form is filled and submitted we get a message “Something went wrong, please try again” instead of the confirmation. The data gets submitted and saved in the backend but the user just does not see a confirmation.
This was tested on two instances of WP previously ie, development and staging and worked without any issue, once the plugin was installed and opt-in created with the same settings on the live site this is when we ran into this problem.
We are not using any email service ie, just saving submission to WP only, and these are our Basic Settings: http://imgur.com/HCNOdvv

I dont believe there are any theme or plugin conflicts as previously mentioned it was working in the dev/stage instances which are a duplicate of the live site, what could be the problem and what would be the steps to troubleshoot if we need to?
