Strange behavior

I’ve used YOP Poll before without problem, but on this one, I’m getting strange behavior.

1. Poll has 10 answers, but only 2 are showing (I’ve tried this with different themes to see if that helps, but no luck).
2. I set the poll to allow multiple answers, but radio buttons are showing instead of check boxes.
3. I set the poll width to 400px, but it’s only 200px wide.

I disabled all other plugins and also cleared browser cache – no luck.

Any suggestions?
