 * I just took a look at your form and it’s displaying correctly on my screen – 
   although with a little custom styling. Did you resolve this issue already?
 * If not, could you please share a screenshot of the issue that you are seeing 
   and let me know what browser you are using so I can try to reproduce and troubleshoot
   this?
