Hi Bkucera,
From the looks of it, your form is missing one ore more fields which are required for the list that is selected in MailChimp for WP > Form, can you please look and confirm which fields your list requires?
You can either mark this fields as “not required” in your MailChimp account or add them to your form using the “Add MailChimp field” tool located on the right side of the screen when editing a form.

i am new…so please bear with me. I don’t know where to find the “fields for the lists” that is referenced above.
Can you send me a screen grab or reference a tutorial for me to follow.
I downloaded the plug in…and added the widget. I don’t know where on my MailChimp account this list would be.
