 * Testing out this plugin and the Submit Request button is not active. You can 
   not press it to submit form. You can also not attach images, button also inactive.
   Rest of the submit form works.
 * Running Flatsome Theme, WooCommerce 5.5.2, WordPress 5.8

   end, kindly check the plugin once with the default standard WooCommerce theme
