 * Typically issues like this may be caused by either a conflict with your theme
   or with another plugin. How I ] usually address this type of issue is to have
   you temporarily switch back to the WordPress Twenty Twelve Theme and disable 
   all plugins except for Awesome Surveys. If that resolves the issue, then slowly
   re-enable features until you find the one that’s causing the conflict.

 * Thank you for the followup. I went through and checked all of my plugins and 
   themes. The problem was with the WPAudio plugin. I was able to work around needing
   this plugin and now your survey plugin is working great. Thank you for you the
   help.
